In order for students to participate in our high school programs like CONS 100, parents are required to complete the Summer Camp Forms Packet.
This secure, electronic packet contains important documents like the Self-Administration of Prescription Medication, Summer Camp Participant Code of Conduct, Photo and Video Release, Pick-Up Authorization, and Medical Authorization to Treat.
Please complete all pages of this electronic packet in advance of the summer program you will be attending.
